New Case Submission

In order to submit a new case, use our New Case Submission Checklist (see Addendum A in the Producer Guide). This document is created with you, the producer, in mind. Easy to read and follow, it outlines what is needed to get your new business submitted in a timely manner.

The following forms are needed:

  • Agent Commission Form (Addendum B)
  • Employer Application* (State Specific)
  • Employee Enrollment Forms (State Specific) or Electronic File Electronic File (Required file layout will be provided)
  • New Case Form

Use the Find a Form or State Availability links available to determine which forms you need for the state you are writing in. For online, web-based electronic enrollment, including call center services, contact your Kemper Benefits Sales Representative for more information.
